Two Tremors?

Post by Marlana »

I just went to a new neurologist today, my previous one moved. He confirmed my OT diagnosis after asking me for a demo. :) I stood up from my wheelchair and instantly had my arms out flailing the air and doing a sort of hula dance and finally dissolving into tears in less than a minute of trying to stay standing.
He said, "Where is that fast tremor you described, that's like a helicopter?" I told him that we didn't get to that part because I am taking a small dose of the gabapentin. I said I mostly just have trouble with balance. He said, "Oh you have another tremor, much slower in your trunk and your head." Now he wants to try increasing the gabapentin, which I've already tried and didn't like, but he wants to go in slower incriments this time. Then when we get it at a reasonable dosage he wants to try the klonapin.
I wanted to do this at least two years ago, but that neurologist didn't want to go to the klonipin because it's so addicting. At this stage adicting seems ok to me.
There is a movement specialist on staff, there, so that may be my next step after the klonapin, maybe before.
Anyone else have two tremors going on? This slower tremor is still only upon standing, so I have no reason to think it is essential tremor or parkinsons.
I would love feedback on klonipin too.

I take Neurontin (900 mg 4 times a day) and have for at least 12 years. It helps a lot. I tried Klonopin and had a bad reaction after two days. I tried it first but since I had such good results with Neurontin (Gabapentin) I haven't tried anything else. Good luck. Oh, I did feel very off when I tried to increase the Neurontin too fast. Hope this helps.

Thanks Patty. Do you mean that you are taking 900 mg total of Neurontin or 900 x 4 which would 3600 mg total? I'm currently taking 600 total and he wants me to work up to at least 900 total, then we'll see.
How did it help you? Would you be willing to be a little more specific? Did it give you more standing time?

That's 3600 mg a day. It helps me not start to shake as soon as I do without Neurontin. It allows me to stand and do things longer before the muscle( I've named it my OT muscle) in my back warns me to stop. If I don't stop, it will go into a spasm and make me stop. I think it helps me go longer and feel more confident than I would without it. One major example of this is, a few years ago I had to have a five level fusion done to my neck. I didn't have my Neurontin for a day when a PT came to my hospital room and wanted me to stand up and go for a walk. Luckily, my husband was there and was holding onto one side of me while the PT held up the other side. I was shaking so bad that I couldn't stand or walk. I told them to give me Neurontin and I would get up and walk. My husband recognized the OT shake and reinforced my request. They started giving me Neurontin and the next day, I got up, walked and went home. I hope this helps.

Thank you Patty. Yes, this is very helpful, and hopeful. I basically have zero standing time. I can get by with a little bit of leaning maybe a minute or two, but not much.

I'm a complex case, interesting, but not typical enough to warrant researching. So, I don't really hold out a lot of hope for getting much standing time back, but comfort, and not panicking would be nice.
I do feel fortunate that at least I do have options when it comes to using a wheelchair. I only need it out side my home, and when I run into having to deal with bathrooms or private residences that are not truly accessible, at least I can get up out of my wheelchair and mange ok, even if I am stressed out afterward.
Thanks so much for your answers, I'm really looking forward to giving the neurontin another try.

I have now been to a movement specialist who has said that I do NOT have two tremors. What that doctor saw was simply me trying to keep my balance while I was standing. This is what I thought, but I was glad to have it confirmed by a movement specialist.
She did help me try to increase my meds to a total of 900 of the gabapentin and I also tried the clonazapam, but I just couldn't deal with feeling so sleepy, so I gave it up after about six weeks. I finally cut back down to 600 mg of gabapentin when I was still fhaving trouble with concentration.
I have to say I had a week of almost normal thinking, it felt nice to be thinking easily and quickly.
But now I"m starting in with Carbidopa-Levodopa, and they are talking deep brain stimulation, but I don't think I'm going to do that. I'm hoping for Lyrica to go generic soon, then I'll try that.

Hello Marlana: Going way back in my history when I was diagnosed with Parkinson's I was given Carbidopa-Levodopa. For 3 yrs. I found my leg were getting worst in many ways. So I started reading as much as I could about Parkinson's and came to the conclusion I did not have Parkinson's and I was mis-diagnosed! I gave up Carbidopa-Levodopa and went searching again for my " I can walk but I can not stand still legs!" When I was finally diagnosed in 2000 with OT I was given clonazepan ... too strong for me and asked if I could try gabapentin and have been on it since . As I have said before it does not make my symptoms go away completely, it just makes my legs feel more bearable. This is what works for me and everyone should trial what works for them. All the best, Gloria

Thanks for your reply, Gloria. My doctor is very clear that this is OT and not Parkinsons. She does not think OT is related to Parkinsons, and I agree. We are running down the list of meds. for me, and since clonapin is too strong for me, we're trying El Dopa now, but I'm not all that hopeful for the meds. I'm still on a tiny (600) total daily dose of gabapentin, that seems to take the edge off.
So, in the meantime, I'm hoping for new meds. :)
