I have a question about this survey. Because none of these factors apply in my case I just checked the last box with none of the other boxes ticked. Is that what one is supposed to do? How else is it possible to indicate that none of these factors seem to apply?

What you did is perfect! If nothing applies, check the last box and hit submit. This is just a basic survey, and a way to start a conversation if any common areas appear. Even a lack of common answers is a result. So it is important that you completed the poll the way you did. It may not be perfect but trying is better than not trying.
