Developing an OT Questionnnaire

Would members find it helpful to have an online questionnaire with the purpose of providing a baseline regarding our OT symptoms and treatment? The questionnaire could be downloaded and completed on an annual basis so creating a record for ourselves which also could be shared with our physicians. I thought that topics might include;

Current use of prescription drugs, dosage and effectiveness.
Mobility issues that we face in our day to day living and the aids that we utilise to assist us.
Engagement in exercise.

So this would not be a survey intended to be scored but a record of your present condition which you could update on a regular basis.

Please let me know if you would be interested in the development of an OT questionnaire. Also content suggestions would be very welcome.

Hi Helen,
I think that would be a very good idea.
IT WOULD BE HELPFUL TO KNOW HOW WE ARE ALL MANAGING THIS DISEASE.
HOPE OTHERS WILL PARTICIPATE.

I think it also would be a good idea. I think the more we share the more we can help each other. I have gotten many ideas and information from other people and would like to see it continue.

Hi Helen,

Good to see you posting on the Forum; I enjoyed meeting you in Rochester, as well as so many others.

Regarding the questionnaire, I think it is a great idea. It would help all of us compare what is going on in our lives, as well as keep a record of how we are changing ~ or perhaps ~ staying the same.

Kathy, I, too, live in FL; perhaps we could meet one day if you would like. If you haven't had the experience of meeting anyone else with OT; it is very comforting to compare notes and share experiences. You can either post here or write me directly at bjlhi@yahoo.com

I agree, a great idea to share and to have something to track our own progress. The medical profession dont seem to be doing much. It seems so unfair when there is research into some diseases that are so much more rare (not that I begrudge them).

Hi Helen,
First of all, thank you for taking time to do this. I would think it would be a good idea to do annually and would be a record of how I am doing and then compare to the previous year's record. Also, it wouldn't hurt to have this information in the event that somewhere down the line a specialist doing research may want to access it.

Dear Dave,
Thank you so much for your helpful input. I have quickly looked at the questionnaire and much of the content is very relevant to us. I will take a closer look later and see how much we can incorporate and what we may need to add.
