Thank you so much for your helpful input. I have quickly looked at the questionnaire and much of the content is very relevant to us. I will take a closer look later and see how much we can incorporate and what we may need to add.

Would members find it helpful to have an online questionnaire with the purpose of providing a baseline regarding our OT symptoms and treatment? The questionnaire could be downloaded and completed on an annual basis so creating a record for ourselves which also could be shared with our physicians. I ...
